A comparative evaluation of four restorative materials to support undermined occlusal enamel of permanent teeth |

AR Prabhakar, P Thejokrishna, AJ Kurthukoti DOI:10.4103/0970-4388.27892 PMID:17065778 The purpose of this study was to test the support to undermined occlusal enamel provided by posterior restorative composite (FiltekTM P60, 3M Dental products USA), polyacid modified resin composite (F2000 compomer, 3M Dental products, USA.), radiopaque silver alloy-glass ionomer cement (Miracle Mix. GC Corp, Tokyo, Japan) and Glass Ionomer cement (Fuji IX GP). To test each material, 20 human permanent mandibular third molars were selected. The lingual cusps were removed and the dentin supporting the facial cusps was cut away, leaving a shell of enamel. Each group of prepared teeth was restored using the materials according to the manufacturer's instructions. All the specimens were thermocycled (250 cycles, 6°C- 60°C, dwell time 30 seconds) and then mounted on an acrylic base. Specimens were loaded evenly across the cusp tips at a crosshead speed of 5 mm /minute in Hounsfield universal testing machine until fracture occurred. Data obtained was analyzed using analysis of variance and Studentized- Newman- Keul's range test. No significant differences were detected in the support provided by P-60, F 2000, Miracle Mix or Fuji IX GP groups. The support provided to undermined occlusal enamel by these materials was intermediate between no support and that provided by sound dentin. Without further development in dental material technology and evidence of its efficacy, restorative materials should not be relied upon to support undermined occlusal enamel to a level comparable to that provided by sound dentin. |

Comparative evaluation of hand wrist radiographs with cervical vertebrae for skeletal maturation in 10-12 years old children  |

A sample of 50 subjects (25 females and 25 males) in the age group of 10-12 years were selected on criteria of normal occlusion and the result showed that cervical vertebrae can be used with the same confidence as hand wrist radiographs to evaluate skeletal maturity, thus avoiding the need for an additional radiograph. |

The effect of tongue appliance on the nasomaxillary complex in growing cleft lip and palate patients |

A Jamilian, R Showkatbakhsh, MB Boushehry DOI:10.4103/0970-4388.27893 PMID:17065780 Midfacial deficiency is a common feature of cleft lip and palate patients due to scar tissue of the lip and palate closure procedure. The aim of this study was to evaluate the effectiveness of the physiological force of the tongue to move the maxilla in forward position. This research has been done experimentally by, before and after treatment following up in private practice. Ten patients (6 female, 4 male) with complete bilateral cleft lip and palate were selected. All of them had Cl III malocclusion with maxillary deficiency due to scar tissue of lip and palate surgery. Their age ranged from 7.6 to 9.8 years. All the patients were delivered tongue appliance to transfer the force of the tongue to maxillary complex. The mean observation time was 13±2 months to achieve positive overjet. Pre- and post-lateral cephalograms were compared to evaluate the skeletal changes with paired t-test. The results showed that after the application of tongue appliance, normal sagittal maxillomandibular relationship was achieved. SN-ANS angle was increased 1.9±1.8 - P < 0.05. Nasolabial angle was decreased 10.3±10.8 - P < 0.03. This study showed that the tongue appliance could transfer considerable force during rest and swallowing period to the maxilla. This method might be considered to improve the deficient maxilla by means of growth modification and redirect concept. |

Determining toothache severity in pediatric patients: A study |

A prospective study of 50 children having different intensities of pain was carried out in the Dept. of Pedodontics, Sareetha Dental College and Hospital. 50 children (aged 3-14 yrs) having different intensities of toothache including normal children (control) were included in the study. Saliva samples were collected and Na+, K+ levels in saliva were measured by using Na+, k+ colorimeter kit. Photographs were taken using Digital camera and VAS was prepared accordingly. Sodium levels decreased with increasing pain intensity and potassium levels increased, facial expressions correlated with Na+, K+ levels. Correlation between Na+, K+ levels and pain intensity exists. Also, VAS is a valid measure for pain. |

An in vitro evaluation of pH variations in calcium hydroxide liners |

The following commercially available materials were investigated: Dycal (LD Caulk); Calcimol (Vocco Products); calcium hydroxide powder (Deepti Products); Calcimol LC (Vocco Products); Lime-Lite (Pulpdent Corporation). Five samples were prepared from each liner. The pH measurements were recorded at time intervals of 1 h, 24 h, 3 days and 7 days after mixing of the liner. The pH variations of each material at the given time intervals were recorded and the means were calculated. Comparison of the mean values at all time intervals with the statistical analysis showed significantly high differences ( P < 0.001) between pH values induced by each material at all time intervals. Among the water-insoluble products, Dycal had the strongest alkaline effect after 7 days interval. The materials that chemically hardened produced higher pH values than materials that hardened by a visible light source after 7 days. |

Oral cleanliness of 12-13-year-old and 15-year-old school children of Sunsari District, Nepal |

A multi-stage random sampling oral epidemiological survey was conducted in private and government, urban, rural town and rural village schools in 15 illakas of Sunsari District, Eastern Nepal. A total of 600, 12-13-year-old and 600 15-year-old school children were examined by trained examiners using the simplified oral hygiene index (OHI-S). The average age-group, debris and calculus index scores were combined to obtain the simplified oral hygiene index (OHI-S). The mean OHI-S scores were compared and evaluated using the parametric t-test for two independent samples. The mean OHI-S for urban 12-13-year-old school children was 0.98 compared to 1.34 for school children of rural towns and 1.44 for school children of rural villages and these differences in mean OHI-S were statistically significant ( P <0.005). In the 15-year-old age group, urban school children had a mean OHI-S score of 1.00 compared to 1.37 for rural towns and 1.43 for rural villages. The variance in the mean OHI-S scores were statistically significant ( P <0.005). The overall level of cleanliness in the school children surveyed was good. Children of urban schools had the lowest scores followed by school children from rural towns and then rural villages. When the mean OHI-S scores were compared with the DMFT scores, there was an inverse relationship between oral cleanliness and dental caries. Frequency of sugar consumption and the availability and affordability of fluoridated toothpaste may be important factors in the development of dental caries than oral cleanliness. |

Aneurysmal bone cyst of the coronoid process of the mandible |

V Rattan, S Goyal DOI:10.4103/0970-4388.27898 PMID:17065785 A rare case of aneurysmal bone cyst (ABC) located in the coronoid process of the mandible in a 12-year-old girl is presented. Treatment consisted of excision of the lesion through preauricular, submandibular and intraoral approach. An access osteotomy distal to second molar region was required to gain access to medial side of the coronoid process. To our knowledge, this is the third case of an aneurysmal bone cyst of the coronoid process of mandible. While examining a patient with a large expansile intrabony jaw cavity with thin peripheral bone, which is filled with blood without presence of bruit, thrills and pulse pressure, the diagnosis of aneurysmal bone cyst should be on top of the differential diagnosis list. Seventy-four to eighty-five percent of aneurysmal bone cysts of jaws occur in 10-20 years age group. Therefore, a pediatric dentist may be the first person to see such a lesion. |

Management of facial trauma in children: A case report |

Below the age of 5, the incidence of pediatric facial fractures in relation to the total is very low ranging from 0.6-1.2%. Maxillo-facial injuries may be quite dramatic causing parents to panic and the child to cry uncontrollably with blood, tooth and soft tissue debris in the mouth. The facial disfigurement caused by trauma can have a deep psychological impact on the tender minds of young children and their parents. This case report documents the trauma and follow up care of a 4-year-old patient with maxillofacial injuries. |
